Abstract
Introduction. Diabetes mellitus (DM) can lead to a variety of microvascular and macrovascular changes, which often culminate in serious clinical complications.
One of them is acute ischemic stroke (AIS). In addition, hyperglycemia is associated with worse clinical outcomes (including higher mortality). The aim of this study was to evaluate differences in the metabolism of lipoproteins and their subfractions in diabetic and nondiabetic patients in the hyperacute phase of AIS (within 24 hours of its onset).
Patients and methods. 46 patients with AIS of the 18 age-matched patients without DM and 18 patients with AIS of 18 age-matched patients with type 2 DM were included in the study. The subfractions of LDL and HDL in serum were determined by the polyacrylamide gel electrophoresis (Lipoprint system).
Results. Patients in the DM-AIS group had significantly (p < 0.05) higher levels of VLDL (27.48 ± 12.01 vs 37.31 ± 19.75 mg/dL), lower levels of HDL (1.23 ± 0.3 vs 1.03 ± 0.24 mmol/L) and imHDL1 (23.53 ± 4.86 vs 19.77 ± 4.46 mg/dL) compared to patients in the AIS group without DM. Of the LDL subfractions, DM patients had significantly higher levels of LDL2 (19.68 ± 12.23 vs 30.00 ± 14.60 mg/dL) and small dense LDL3-7 subfractions (1.55 ± 0.56 vs 8.62 ± 2.96 mg/dL), which positively correlated with the levels of triacylglycerols (TAG) in both monitored groups (p < 0.001). TAG levels also positively correlated with the levels of small HDL8-10 subfractions (p < 0.05).
Conclusion. Our results indicate a more atherogenic lipoprotein profile in patients with type 2 DM compared to nondiabetic patients in the hyperacute phase of AIS (Tab. 2, Fig. 4, Ref. 18). Text in PDF www.lekarskyobzor.sk.
